About HIPAA
The Health Insurance Portability and Accountability Act of 1996 (HIPAA) is a federal law that protects the privacy and security of your personal health information.
At Nurture Lactation, LLC, we honor your trust by safeguarding your information and using it only as allowed by law. As lactation consultants and educators, we keep records of our sessions to support your care, and all personal health information is kept strictly confidential.
Your Rights Under HIPAA
Access to Records
You may request a paper or electronic copy of your health records at any time. We will provide these within a reasonable amount of time.
Changes to Records
If you believe your record contains inaccurate information, you may request a correction. We will review your request and make updates when appropriate.
Disclosure of Records
You have the right to know who your records have been shared with and why, going back up to six years. One disclosure list per year is free; additional requests may incur a reasonable, cost-based fee.
Restriction of Sharing
You may request limits on how your information is shared. While we will consider all requests, there are times when we are required by law or clinical need to share certain information.
Access to This Notice

Designation of a Representative
If someone has legal authority to act on your behalf—such as a legal guardian or a medical power of attorney—that person may exercise your privacy rights. We will verify their authority before taking action.
Filing a Complaint
If you believe your privacy rights have been violated, you can file a complaint using the contact information below. You will not face retaliation for filing a complaint.
How We May Use and Share Your Information
For Treatment
Your health information may be shared with other healthcare professionals involved in your care to coordinate services and support your feeding goals.
For Billing and Payment
We may use your information to bill and collect payment from you, your health plan, or other authorized payers.
For Practice Operations
We may use your information to improve the quality of care we provide, to follow up on your progress, or to contact you about services that may benefit you.
To Comply With the Law
In certain situations, state or federal law may require disclosure of specific health information (for example, for workers’ compensation claims).
For Legal or Government Requests
We may share your information in response to law enforcement requests, government oversight, or legal proceedings (e.g., court orders or subpoenas).
Information Shared Only With Your Written Permission
We will never share your information for:

If you would like your information shared for any other reason, you must submit a written request specifying what information should be shared, the purpose, and to whom it should be sent. We will only release the minimum necessary information.
